Immunizations. As a healthcare provider, you will interact with a variety of patients as part of the educational process in the College of Dentistry. Because of this patient contact, and for your own well-being, it is important to have a complete and accurate medical history and immunization record. As a condition of enrollment, the immunization form must be returned to OSU Student Health Services two months prior to the beginning your internship. Your requirements for measles, mumps, rubella, varicella and tetanus/diphtheria must be complete when the form is submitted. The tuberculosis test and influenza vaccine are annual requirements and should be current through the duration of your internship.  If you have this test done at OSU Student Health Services it will be added to your health record; if you have the test done elsewhere you should fax the results to Student Health Services at 614-292-6001. A list of the immunizations required for the MS DH program is listed on our Immunization Requirements page.
